Fridell, JA, and others: The Elusive Definition of Type 2 Diabetes in the Context of Pancreas Transplantation: Summary of the International Pancreas and Islet Transplantation Association Pre-Congress Meeting
The pancreas transplant field strives to identify candidates among those with features of T2DM who may benefit from transplantation. Clearly, it appears imperative to uniformly and consistently communicate about these patient populations for the implementation of policies and assessment of study outcomes, and alternative therapies. This report presents definitions and listing Guidelines for Pancreas Transplantation in Type 2 Diabetes Mellitus.

Rahman, M, and others: The downregulation of LKB1 in small extracellular vesicles elucidates the downstream mechanisms driving acute lung allograft rejection.
Acute cellular rejections present a risk factor for chronic lung allograft dysfunction (CLAD). Here, the authors present liver kinase B1 (LKB1) as a key regulator in lung transplant alloimmunity. In detail, they identify LKB1 as a key suppressor of inflammation, cell growth, and division in lung cells. This master regulator has the potential to serve as a novel and dynamic biomarker and novel therapeutic target.

Tanaka, T and others: Incorporating MELD Trajectories into Liver Allocation: Assessment of a MELD-Informed Model
Using OPTN data, this study evaluated whether MELD score trajectories improve transplant prioritization. MELD velocity (MELD/day) was strongly associated with waitlist mortality and significantly improved risk reclassification beyond static MELD. These findings support incorporating MELD dynamics into future allocation frameworks to better identify high-risk patients and promote equitable access to transplantation.

Wiebe, E and others: Declining All-Cause Allograft Survival in the Modern Era – a 50-year single-center consecutive cohort study
This analysis of 2,076 kidney transplants (1969–2024) provides up to 50 years of directly observed outcomes, revealing that death-censored survival has plateaued while overall allograft survival has declined in the modern era. Era-stratified Cox models show that this reversal is driven by rising donor and recipient age and increasing comorbidities. Death with functioning graft now accounts for >60% of graft losses, with infectious deaths nearly doubling.

These findings have immediate relevance for contemporary transplant programs.

T-cell Activation and Increased Levels of Cytokines in Children After Liver Transplantation—A Potential Association With Increased Susceptibility to Food Allergy Development

Pediatric liver transplant (LT) recipients are at increased risk of developing a food allergy (liver transplantation-associated food allergy [LTFA]). The underlying immune mechanisms are unknown. We evaluated the circulating T- and B-cell subsets and serum cytokine profiles of LT recipients with and without LTFA in a cross-sectional cohort study of 43 LT recipients, 8 of whom had LTFA. The T- and B-lymphocyte subsets were analyzed by flow cytometry, and the serum concentrations of 15 cytokines were measured. For comparison, nontransplanted food allergic controls and healthy controls were recruited.

Clinical Impact and Cost-effectiveness of Everolimus Combination Therapy in Liver Transplant Recipients: Focus on the Risk of Cancer, Infection, and Renal Function

Post–liver transplant complications—cancer, infections, and renal dysfunction—impose substantial clinical and economic impact on recipients. This study evaluated clinical impacts and cost-effectiveness of adding everolimus (EVR) to a calcineurin inhibitor (CNI)-based regimen in liver transplant recipients, aiming to identify how co-occurring major complications affect cost-effectiveness.

Renal Function Decline After Vascularized Composite Allotransplantation

Chronic kidney disease (CKD) is a common complication of nonrenal transplantation, influencing recipients’ morbidity and mortality. The incidence of CKD varies depending on the transplanted organ. Recipients of vascularized composite allotransplantation (VCA) need lifelong immunosuppression similar to that of solid organ transplantation. This study investigated renal function in VCA recipients.

Management of Norovirus Infection in Solid Organ Transplant Recipients: A Scoping Review

Complex abdominal wall herniations present major reconstructive challenges, particularly in patients with contaminated fields or previous failed repairs. Synthetic meshes carry substantial infection risk, whereas biological meshes are expensive and exhibit inconsistent long-term strength, leaving many patients without dependable options. This study investigates the feasibility, safety, and immunologic profile of nonvascularized rectus fascia allotransplantation (NVRF-alloTx) for abdominal wall reconstruction in immunocompetent individuals.

T-Cell Engager–Mediated HLA Antibody Depletion before Kidney Transplantation

July 29 - HLA sensitization remains a major barrier to successful kidney transplantation, with prolonged waiting times for transplantation and increased risk of rejection despite desensitization and tailored organ allocation. B-cell maturation antigen (BCMA)–directed T-cell engagers that deplete B cells and plasma cells may offer a new approach to desensitization.Here, we report successful use of teclistamab in a patient with profound HLA sensitization.

July 29 - A machine that keeps organs alive outside the body could save four in every five donated lungs that currently go to waste in England and Wales. Draft guidance from the National Institute for Health and Care Excellence (Nice) recommends that the XVIVO perfusion system be used in the NHS for a four-year period to assess its effectiveness and value for money.

July 29 - Among patients with high-risk hematologic malignancies who underwent hematopoietic cell transplantation with posttransplant cyclophosphamide, mismatched unrelated donor (MMUD) transplantation was associated with lower relapse rates than matched related donor (MRD) transplantation while showing comparable disease-free and overall survival, according to a retrospective cohort study by Mehta et al published in JAMA Network Open. The relapse benefit was accompanied by a higher risk of chronic graft-vs-host disease (GVHD).

July 30 - An international research team led by the Medical University of Vienna reports on a new treatment strategy that has made a kidney transplant possible for a patient with no realistic chance of receiving a suitable donor organ. As the case study demonstrates for the first time, the use of a new drug from the field of cancer medicine can achieve a substantial and sustained reduction in antibodies against potential transplants, a level not previously attained. This significantly improves the prospects of a successful organ transplant even in cases with a particularly unfavourable initial immunological profile. The results have recently been published in the highly respected New England Journal of Medicine and could open up new perspectives in transplant medicine.

July 29 - Specialized cells in the human body make biological trade-offs to perform certain jobs. To support the large, hardworking hearts that power other organs, heart cells have evolved to be extremely efficient and resilient. Because the adult human heart cannot repair itself the way skin does, scientists have studied ways to "reprogram" heart cells to regenerate after heart attacks. Heart cells have very stable identities, however, and are resistant to reprogramming.

July 30 - Remdesivir was shown to reduce the risk of all-cause graft loss and cardiovascular events in adult kidney transplant recipients with symptomatic COVID-19 who’d received at least 3 consecutive days of the antiviral therapy. The recent study’s findings underscore the importance of evaluating antiviral treatments for underrepresented patients in COVID-19 clinical studies, such as kidney transplant recipients.

July 28 - Kidney transplantation remains the preferred treatment for kidney failure because it improves survival and is more cost effective than long-term dialysis. However, new research using a large US electronic health record database found that substantial patient attrition occurs long before transplantation, highlighting persistent barriers throughout the transplant evaluation process.
