Among the topics covered in this issue in kidney transplantation include the use of tocilizumab (anti-IL-6) to treat chronic active antibody-mediated rejection, and the personalized management of transplant outcomes using an Adjustable Graft Failure Scoring system. In living kidney transplantation, a group from Canada reports on substantial barriers that limit pre-transplant donor eligibility evaluations; from Australia, an Australian Matchability Score is introduced to better balance kidney transplant accessibility and HLA matching in deceased donor allocation. Direct Procurement and Perfusion Using Hypothermic Oxygenated Perfusion for DCD Cardiac Allografts in North America
Donation after circulatory death (DCD) is a viable avenue for recovery of cardiac allografts for transplantation. Previous methods have utilized direct procurement and perfusion (DPP) using commercially available normothermic perfusion systems or in situ normothermic regional perfusion. A novel portable hypothermic oxygenated perfusion device (HOPE) (XVIVO Perfusion) has been developed and is in clinical trials for preservation of cardiac allografts. Here, we report the first North American experience using a HOPE preservation device for DPP recovery of DCD cardiac allografts and detail our experience and early patient outcomes.
Perfusate Biomarker Comparison During Renal Hypothermic and Normothermic Machine Perfusion: Do These Techniques Provide Similar Insights?
Hypothermic machine perfusion (HMP) and normothermic machine perfusion (NMP) are increasingly used in renal transplantation. Both techniques enable pretransplant organ viability assessment through biomarker measurements in the perfusion solution. This study examines similarities and differences in biomarker release during HMP and NMP, focusing on well-established biomarkers alongside functional markers in porcine and discarded human donor kidneys.

Pretransplant Immune Checkpoint Inhibitor Treatment for Intermediate-advanced Hepatocellular Carcinoma in Living Donor Liver Transplantation
The use of immune checkpoint inhibitors (ICIs) as a downstaging or bridging therapy for liver transplantation (LT) in patients with hepatocellular carcinoma (HCC) is gaining momentum. However, evidence regarding the feasibility and safety of pre-LT ICI therapy remains limited and controversial. This study presents our experience with patients with intermediate-advanced HCC who were downstaged using regimens incorporating ICIs and subsequently underwent living donor LT.
