Just Released - Transplantation Direct - April 2026

We are pleased to introduce the April issue of Transplantation Direct, covering interesting topics across the field. Starting with kidney transplantation, we have a report from South Africa analyzing how access to dialysis and transplantation in low-mid income countries critically affects communities. A more diagnostic study looks at correlations in kidney allograft gene expression data in relation to histological phenotypes. In liver transplantation, there is a highly relevant retrospective investigation looking at post-transplant impacts in patients taking, or not taking, GLP1 receptor agonists; another article compiles a large series of surgical cases where calcified portal vein thromboses were successfully removed before portal vein anastomosis during transplantation. Dialysis is again the topic in a lung transplantation article, where investigators use risk factor planning and control to reduce the need for post-transplant kidney replacement therapy. In pediatric heart transplantation, a strategy is proposed for reducing the need for invasive heart rejection monitoring in pediatric patients. For those interested in pancreatic islet transplantation, we have a report comparing islet isolates from DBD versus DCD donors. This issue additionally features several articles on organ preservation techniques, including experimental porcine studies on the effects of varying hemoglobin levels during ex vivo heart perfusion, on the use of red blood cell-stabilizing agents to reduce hemolysis during normothermic machine perfusion of livers, and on the use of alkaline phosphatase supplementation during kidney machine perfusion performed at different temperatures. Also related to organ donation and preservation, we have an article on factors guiding in-transit and back-to-base machine perfusion strategies for liver transplantation. On the topic of infectious diseases in transplant recipients, two randomized trials are being reported on; one where IVIg was tested to treat BK polyomavirus infections, and another where IVIg was used to treat infections in patients with secondary antibody deficiencies. The Impact of Recipient HIV Status on Kidney Transplant Outcomes in the United Kingdom: Have Previous Studies Overestimated Risk?
Despite increasing literature on kidney transplantation in people with HIV, studies have largely overlooked the confounding impact of ethnicity and socioeconomic deprivation. This study aimed to assess posttransplant outcomes in HIV-positive recipients, adjusting for these key factors.
The Kidney–Immune Axis: How Erythropoietin Regulates Tolerance and Rejection
Erythropoietin (EPO) is classically viewed as a kidney-derived hormone essential for erythropoiesis, yet accumulating evidence places it at the center of complex interactions between the kidney and the immune system. The kidney is both a frequent target of immune-mediated injury and an organ with distinctive tolerogenic properties, capable in experimental models of inducing acceptance of otherwise rejection-prone allografts. Herein, we review experimental and clinical data showing that EPO, through its homodimeric EPO receptor (EPOR) or heterodimer EPOR/CD131 receptors, shapes both adaptive and innate immunity in ways that are directly relevant to tolerance and rejection.

Quality Assessment of Discarded Human Kidneys Using Normothermic Perfusion
Ex vivo normothermic perfusion (EVNP) allows for comprehensive functional assessment; however, current scoring systems are validated only for short perfusion durations. This study investigates the utility of the EVNP quality assessment score for a 6-h period to predict viability in discarded human kidneys.
Alkaline Phosphatase Supplementation During Hypothermic and Normothermic Machine Perfusion Improves Metabolic Dysfunction in a Porcine Model of Donor Kidney Preservation
Kidney preservation techniques, such as hypothermic machine perfusion (HMP), improve graft outcomes in deceased-donor kidney transplantation by pausing graft metabolism and ameliorated ischemia–reperfusion injury (IRI) but do not completely eliminate injury. Alkaline phosphatase (ALP) has been postulated to reduce IRI-induced kidney injury through the conversion of extracellular adenosine triphosphate into adenosine. This study aimed to evaluate whether ALP offers protection during deceased-donor kidney storage.
